Welcome to our office! Thank you for trusting your healthcare needs to our group. It is our mission to take the best care possible of you and providing world class medical service.
As you are a new patient in our group, we must get some information from you. Please take a few moments of your time to fill out the "
New Patient Forms" Once you have completed these forms, please bring them to your first appointment with your cardiologist.
This will save time at registration and provide us all of the information we need to make sure we provide the service our patients expect from us.
Existing Patients / Follow My Health
Thank you for beginning the process of enrolling in Follow My Health. This is a innovative tool offered to our patients so they may access their information at any time. With Follow My Health, you may access your secure online medical record with Interventional Cardiology Medical Group, Inc., and any other provider that uses the same portal.
Follow My Health allows you to send messages to your cardiologist, view normal test results, request prescription renewals, request an appointment around your schedule and keep current with your copays and any balances owed.
Please follow the instructions below, be sure your cardiologist's assistant has your
current email address, and remember that Follow My Health
IS NOT to be used for urgent needs. For any medical emergency, dial
911.

You Received an Invitation Email
In this email there will be a link contained where you will be prompted with the appropriate Invite Screens.
1. The landing page is the first screen you will see. You must choose the appropriate action:
Create an Account: choose this option if you do not have an existing Follow My Health (FMH) account with any other healthcare organization.
Add This Connection: Choose this option if you do not have an existing FMH account with another healthcare organization.
2. You will be prompted to log in with any of the four authentication providers:
- Facebook
- Google
- Yahoo
- Microsoft Live ID
Choosing one of those four providers to log in with simply authenticates you to FMH. In no way is any information shared from FMH to them or from them to FMH. FMH does not store your login credentials and does not have access to your password.
3. Upon choosing the Login method and successfully logging in, you will be taken through the invitation process.
- The INVITE WIZARD displays. The bar across the middle top tells you what step you are on in the acceptance process. You must click the "next" button to continue.
4. Connect to Your Account
- Enter the invitation code you received when you signed up for a FMH account.
- This will be either the last four digits of your social security number or the 4 digit year of your birth.
- If you do not know your invite code, contact your doctor's assistant to obtain it.
5. Accept the Terms Of Service
- To print the terms of service click the "print" icon
- Here, you also have access to the Privacy Policy that you may also print by opening the policy
6. Connect To Your Account
- You must accept the Release of Information.
- The Release of Information allows FMH to obtain your medical information from your cardiologist and store it on your behalf
- If you decline this step, your medical information may not be obtained from your cardiologist
7. Almost There!
- Upload your health record when prompted to do so
- Once the upload is complete, you will be taken in the Home screen of the Patient Access area where you are free to explore all of its benefits to you, the patient.
Coumadin Clinic
Our Coumadin Clinic is coordinated to allow our patients to optimize their anticoagulation medications Coumadin, or in its generic form warfarin. Coumadin affects your blood's ability to clot. When first starting Coumadin, patients will need to have their blood checked frequently until their dose is regulated. The frequency of testing can gradually be decreased after therapy is stable.
The frequency will also depend on other factors, such as your clinical condition, diagnosis, and your Cardiologist's recommendations. It is very important to your health to follow your physician's advice on how often you should come to the Coumadin clinic.
Our clinic will provide:
- Immediate test results using a finger-stick method
- Managing Coumadin/warfarin dosing
- Monitoring and patient evaluation
- Education
Our on-site clinic is open during working hours:
- Monday – Thursday: 9:30 am – 11:30 am
- Monday – Thursday: 3:00 pm – 4:00 pm
- Friday appointments are not available
