The Fine Print

  • Pricing is determined by the average rates for all locations included in the Silver State Smile Club plan schedule.
  • Most procedures not listed will be discounted by 20%. Confirm unlisted procedures with your Silver State Smile Club location before appointments.
  • Prices are subject to change and are not valid with any other discount or insurance.
  • A $10 office visit fee applies to every Silver State Smile Club location visit.*
THIS PLAN IS NOT INSURANCE and is not intended to replace health insurance. This plan does not meet the minimum creditable coverage requirements under M.G.L. c.111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. The list of participating providers is at www.silverstateclub.com. A written list of participating providers is available upon request. You may cancel within the first 30 days after the effective date or receipt of membership materials (whichever is later) and receive a full refund, less a nominal processing fee.


Terms & Conditions for SSSClub Membership Plan

  1. Membership Duration: The SSSClub Membership Plan is valid for one year from the date of enrollment.
  2. Payment Terms: Membership fees are paid annually, in a single lump sum at the beginning of each membership year.
  3. Automatic Renewal: The membership will automatically renew annually unless canceled by the member prior to the renewal date.
  4. Cancellation Policy: Members may only cancel their membership by providing a 30-day notice prior to the annual renewal date. Cancellations made after this period will not be effective until the following annual renewal.
  5. Non-Refundable Fees: Annual payments are non-refundable, and no partial refunds will be provided for any unused portion of the membership year.
  6. Service Availability: Membership benefits are only available to active members who have paid the annual fee.
  7. Modification of Terms: The dental practice reserves the right to modify the terms and conditions of the membership plan. Members will be notified of any changes at least 30 days in advance.
  8. Governing Law: These terms and conditions are governed by and construed in accordance with the laws of the state in which the dental practice operates.

By enrolling in the SSSClub Membership Plan, members agree to these terms and conditions.
