Preparing for Your Appointment

To assist us in providing you with the best possible care, please bring the following with you on the day of your appointment:

• A referral slip from your doctor.
• A list of medications you are presently taking.
• A government issued photo ID.
• Complete insurance information including the member’s name, social security number, date of birth and name of the employer.
• Minor children under the age of 18 must be accompanied by a parent or legal guardian.

Please arrive 10-15 minutes early for your first appointment to complete your registration with our office.
