Program Guidelines
- Plan goes into effect immediately if spaid in full, otherwise 6-month waiting period if paying monthly
- Plan has NO maximums, deductibles, claim forms, waiting periods, missing tooth clauses, or pre-existing condition clauses
- May enroll and use it in conjunction with dental insurance for non-covered services
- Plan CANNOT be used for take-home whitening or retail products sold in office
- No refunds or rollovers will be issued at any time if participant fails to use the dental plan
