Unlike your typical dental insurance plan, our hassle-free Smile Saver Plan has transparent pricing, as well as NO yearly maximums, NO deductibles, NO claim forms to fill out, NO pre-authorizations, and NO waiting periods that limit the care you receive.

PRICING

$405 per year for an adult patient
$345 per year for a child patient (up to age 18)

Includes (per year):

  • 2 full oral examinations (new patient or re-care exam)
  • 2 preventative cleanings*
  • All radiographs (full set) needed to accurately diagnose oral conditions
  • 1 emergency visit and required radiographs for that visit
  • 10% off any other treatment (including implants, tooth whitening, sedation and more!)

Our Smile Saver Plan includes a minimum of $700 in value ‡ !

*Depending on your periodontal status, two additional cleanings per year (up to 4 per year) may be necessary to restore teeth and gums to a normal state. As this is specialty and not preventative treatment, the 10% discount applies.

Terms and Conditions

  • The Brockport Dental Smile Saver Plan is considered active for 12 consecutive months starting the day the plan is purchased.
  • Payment is due in full on the first day that services are rendered (“Effective date”), and pre-payment towards the plan is not accepted. The first day of service can include any dental visit, whether it is a preventative visit or emergency, etc. The Effective date will be recorded in our ledger.
  • The member is responsible to maximize benefits by arranging the appropriate appointments within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund.
  • If members wish to renew plan, renewal payment is due no earlier than 12 months after the Effective date.
  • Membership rates may be reviewed and adjusted on an annual basis, with prior notification before renewal payment is made.
  • This savings plan is not insurance and cannot be used in conjunction with another dental plan or dental insurance.
  • This savings plan cannot be used in conjunction with other office promotions or bundle pricing available.
  • This plan is non-refundable, and no credit or refund can be issued at any time if a member decides to no longer utilize the membership for any reason, including switching dental offices.
