At Alvarez Dental, your first appointment includes a thorough exam and gathering any information that our dentist must be aware of in order to provide care specifically to your oral health. We understand that all patients are different and require individualized care. 




Financial Policy


Payment is expected and due at time of service. We accept cash, checks, Apple Pay, Visa, Mastercard, American Express and Discover. Additionally, we accept Care Credit but this must be applied for before treatment.

If you do not have insurance, payment is due in full at time of service.

For patients with insurance coverage, we ask that you provide a current driver's license and insurance card at each office visit to ensure eligibility. If insurance verification and coverage cannot be determined prior to the visit, full payment will be requested at the time of service.

Please notify our office if your insurance has changed prior to your appointment.

Please be advised that the eligibility and benefit information supplied by your insurance company is only an “estimate” and is not a guarantee of payment by the insurer. You are ultimately responsible for all charges. We cannot guarantee that any benefit “estimated” by your plan will be paid once a claim is filed. Actual benefits are subject to all plan terms, definitions, limitations and exclusions in effect on the date of service. Alvarez Dental will submit your claim to your insurance company for services performed by our providers at our facility; however, if we are unable to collect from the insurance company, it is the patient's responsibility to pay for any and all services provided.

The insurance contract is an agreement between you and the insurance company. While our staff does our best to pre-estimate any necessary co-pay for your convenience, it is the ultimate responsibility of the patient to understand all policy terms set forth by their insurance company. Many insurance companies do not provide enough necessary information to our office for us to accurately estimate your out-of-pocket costs. We can provide our fees at anytime while you are present in the office prior to any services rendered.

Cancellations and missed appointments: We request that you give our office a minimum of 24-48 hours notice, based on appointment type, if you need to cancel or reschedule an appointment.

Collections: Failure to pay your balance within 90 days will result in your account being turned over to a collection agency and reported to a credit bureau.
