Program Guidelines
- Cannot be combined with any other offer
- Cannot be used in conjunction with outside financial services
- Membership card will not be issued. The plan’s effective date will be on file
- Patient’s portion of the bill is due the day of service
- This plan is non-refundable
- Missed appointments or cancellations with less than two business days notice are counted as an appointment
- Must remain a plan member for the duration of treatment to retain discount plan benefits
