New Patient Forms
To provide you with the most efficient new patient experience, we ask that you arrive at least 15 minutes prior to your first appointment. Our dental team will take comprehensive records and address any concerns or questions you may have.
Patient Registration
You may preregister with our dental office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Your First Visit
Your first visit with us may include:

During your visit, we will answer any and all questions about your oral health. Our dental team will provide a detailed financial consultation to ensure your treatments are timed effectively and affordably.

We use electronic charts for patient’s records. These records are encrypted and stored electronically. We also use an encrypted email system, so our patient’s information is sent securely via email.

Financial Policy
For your convenience, we accept cash, checks, Visa and MasterCard. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Most insurance companies will respond within four to six weeks. Please call our dental office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
